The fundamental difference lies in how the laser accesses the cornea. LASIK creates a thin surface flap that is folded back to allow a secondary laser to reshape the bed, while SMILE uses a single femtosecond laser to cut a tiny, keyhole-sized incision and remove a microscopic tissue disc (lenticule) right out from inside the undisturbed cornea.
